Students Age 12 and older
  • COVID-19 Vaccine: Because of COVID-19, it is important that we have an updated Confidentional Health History Update Form for every student for the 2021-22 school year. If your child has received the vaccination for COVID-19, please check the “Other” box and provide the vaccination information in the space below or you may send in a copy of the vaccination card for the nurse to include with your child’s immunizations. This form may be submitted electronically through the PowerSchool Enrollment update or via hard copy to the school nurse before the start of the school year. There is no diocesan mandate for your child to receive the COVID-19 vaccination however knowing who has received it will be helpful in simplifying the contact tracing process as needed throughout the school year. If your child receives the vaccine after the start of the school year, please contact the school nurse with the updated information.
Parents who choose not to immunize their child must complete a Certificate of Religious Exemption Form. This form must be notarized.
