Virginia State Required Immunizations
Students entering Pre-K, Kindergarten, or transferring to St. Louis Catholic School must have a completed VA School Entrance Health Form, to include the child’s physical and the minimum immunization record dated within one calendar year of the first day of school, in his or her health file. Students returning in grades 1st-8th should have a Confidential Health History Form completed with updated immunization records (if applicable) the first day of school. Please ask your physician for a copy to provide to the school health office.
 
Rising Seventh Grade Students
  • Tdap: A booster dose of Tdap vaccine (Tetanus, Diphtheria, and Pertussis) is required for students entering the 7th Grade.
  • Meningococcal: Effective July 1, 2021, a minimum of 2 doses of MenACWY vaccine. The first dose should be administered prior to entering 7th grade. The final dose should be administered prior to entering 12th grade.
  • Human Papillomavirus Vaccine (HPV): Effective July 1, 2021, a complete series of 2 doses of HPV vaccine is required for students entering the 7th grade. The first dose shall be administered before the child enters the 7th grade. After reviewing educational materials approved by the Board of Health, the parent or guardian, at the parent's or guardian's sole discretion, may elect for the child not to receive the HPV vaccine.
All Students
  • COVID-19 Vaccine: Because of COVID-19, it is important that we have an updated Confidentional Health History Update Form for every student for each school year. If your child has received the vaccination for COVID-19, please check the “Other” box and provide the vaccination information in the space below or you may send in a copy of the vaccination card for the nurse to include with your child’s immunizations. There is no diocesan mandate for your child to receive the COVID-19 vaccination however knowing who has received it will be helpful in simplifying the contact tracing process as needed throughout the school year. If your child receives the vaccine after the start of the school year, please contact the school nurse with the updated information.
Exemptions
Parents who choose not to immunize their child must complete a Certificate of Religious Exemption Form. This form must be notarized.
