SchoolPass & Daily Wellness Check
Parents are required to fill out the Wellness Check through SchoolPass for each of their children enrolled at St. Louis School on a daily basis. If you have trouble accessing your child's profile on SchoolPass, you may print and send in the Daily Health Questionnaire below.
**If you answer ‘yes,’ to any questions on the Wellness Check, please keep your child home and email the school nurse at [email protected]. The nurse will contact you to approve your child’s attendance or ask you to keep your child home and consult with their pediatrician per the VDH algorithm.
