PLEASE NOTE - This form is for those families who opted to purchase milk on a Trimester basis or who are placing a new order. If you ordered for the full school year, please do not complete this form.

The cost of milk for the Trimester 3 is $60. All payments will incur processing fees.

Complete the form and payment below to place a milk order for your child(ren). If ordering for more than one child, please fill in a separate entry for each student (up to 4 students per form). Remember to include your child's first AND last names!

Milk orders must be placed no later than March 8, 2024.

ELECTRONIC ORDERS & PAYMENTS ONLY; cash/checks will not be accepted.

(Purchasing milk is optional. If your child prefers another beverage with lunch, we ask that it be limited to water or a juice box. Sodas, sports drinks, and glass bottles are not permitted in the cafeteria.)
