PLEASE NOTE - This form is for those families who opted to purchase milk on a Trimester basis or who are placing a new order. If you ordered for the full school year, please do not complete this form.

The cost of milk for Trimester 3 (March - end of school year) is $60. All payments will incur processing fees.

Complete the form and payment below to place a milk order for your child(ren). If ordering for more than one child, please fill in a separate entry for each student (up to 4 students per form). Remember to include your child's first AND last names!

Milk orders must be placed no later than February 28, 2023.

(Purchasing milk is optional. If your child prefers another beverage with lunch, we ask that it be limited to water or a juice box. Sodas and glass bottles are not permitted in the cafeteria.)
